  • Moving Forward Rehabilitation

    Moving Forward rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.

    Our person-centered program is tailored to the individual. Moving Forward includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.

    Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.

    Returning home as quickly as possible is always the desire. However, returning home without the proper rehabilitation can greatly increase the chance of seniors experiencing a setback resulting in rehospitalization. Prior to returning home, our team, in partnership with the patient and family, will conduct a comprehensive in-home safety evaluation to ensure patients can successfully navigate their home with confidence.

    No matter what your age or health level, our clinicians are skillfully trained and committed to your success. They take time to get to know and understand what motivates you. Based on proven methodologies, you can count on our clinical team to provide the latest treatments, with respect and compassion. Your health and happiness are always our priority.

    Physical Therapy focuses primarily on independent mobility, regaining strength and balance, using proper body mechanics and energy saving techniques to better perform mobility tasks.

    Occupational Therapy promotes independence with daily living activities like bathing, eating, dressing, self-care and home management tasks. Participants gain strength and coordination while learning to conserve their energy.

    The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.

    We monitor our senior rehabilitation outcomes and use the data to consistently improve our services.

  • Auguste’s Cottage Memory Care

    Auguste’s Cottage is a structured, research-based memory care program. We utilize a person-centered, wellness-based model of care that incorporates stimulating daily activities to sustain independence and promote a healthy spirit. Auguste’s Cottage is a self-contained center within our care community. The concept is to focus on maintaining a secure, home environment where residents have a sense of belonging and meaning each and every day. We offer:

    • A Meaningful Day activity program that builds on the four domains of wellness
    • An emphasis on holistic and innovative care philosophies
    • A reminiscent and dementia supportive living environment

    Auguste’s Cottage is a self-contained center within our communities. We surround residents with artwork and special personal items from their past to help foster a sense of belonging and cultivate feelings of security. Plants adorn the halls and pets visit frequently. This protected environment fosters dignity and supports autonomy by reducing challenges.

  • Long-Term Care

    Long Term Care is a great option for those who struggle with daily tasks and can no longer be cared for at home due to illness, injury, frailty or other limitations. Under the guidance of an attending physician, Long Term Care is provided in our communities by licensed nurses, specially-trained health care aides and a full interdisciplinary care team.

    Long Term Care includes a variety of services, including social services and activities as well as physical, occupational and speech therapies to assist our residents in maintaining the highest level of independence possible. Our settings are home-like, cheerful, soothing and secure to ensure the highest quality of life for our residents.

    Along with private pay, ASC is proud to accept a wide variety of payment options including Medicaid for qualified applicants. Our Medicaid specialists can assist you and your family through the application process.

  • Respite Care

    While it’s natural for caregivers to allow their own needs to get pushed aside, it’s important for them to realize that they deserve a break from their responsibilities in order to properly care for themselves.

    This is where senior respite care can make a difference in a caregiver’s life. Respite stays are an option for those who need respite care services for elderly friends or family on a short-term basis. This allows home caregivers the ability to travel, start a new job, recuperate from an illness, or pursue other interests. Many find respite stays are the perfect option for recovery from surgery or hospitalization.

    Should a short stay require extended care, we can accommodate that, too. We carefully examine the needs of our respite guests and provide the level of care required to make their stay comfortable and enjoyable.   • Hospice Care

    Hospice and palliative care for seniors is designed to provide comfort, care, and pain relief for those facing terminal illness. Hospice care involves a team approach to supporting patients and their families with expert medical care and pain management, along with meeting the spiritual and emotional needs of both the patient and the family.

    Hospice and palliative care represent a compassionate approach to end-of-life care, assuring the best quality of life and allowing your loved ones to live as comfortably as possible. American Senior Communities partners with local hospice companies to offer comprehensive hospice services, including skilled nursing care, respite care, spiritual and bereavement counseling, medical equipment and supplies, social services, and pharmacy services. By assembling an interdisciplinary team of physicians, nurses, aides, volunteers, social workers and clergy, we are able to serve the special needs of hospice patients and their families.

  • Ventilator Care

    The Ventilator Care program at American Senior Communities offers a comprehensive approach to caring for individuals who require specialized equipment that pumps oxygen into the lungs. Ventilators help a resident breath and ensure the appropriate amount of oxygen is supplied to the blood stream.  Our highly skilled and specialty trained interdisciplinary teams include a Respiratory Therapist, Pulmonologist, RNs and other skilled and compassionate caregivers.

    Under the direction of a Pulmonologist, we customize goals to wean patients off the ventilator, whenever possible.  However, each resident is treated individually with a very specific care plan to treat the whole person.  Our holistic approach is designed to improve comfort, safety, strength, endurance, participation in daily activities and overall quality of life.  Some of the components of the program include:

    • Respiratory therapy 24-hours a day.
    • Advanced Cardiac Life Support (ACLS) certified staff.
    • Local Pulmonologist available for additional care and consultation.
    • Speech, occupational and physical therapy availability to optimize functional recovery.
    • Systems in place to alert when oxygen levels are not optimal.
  • Advanced Respiratory Therapy

    American Senior Communities proudly offers a specialized program designed to help patients with respiratory conditions reach their highest practicable level of functioning while reducing the risk of re-hospitalization. This person-centered, outcome-based program includes Pulmonologist consultation, on site Respiratory Therapists, disease management education, social services support, and post-skilled integrated care management. The plan of care is customized to the needs of each individual.

    Our focused respiratory program allows participants to improve their quality of life by:
    • Teaching self management of the disease
    • Increasing physical endurance and activity level
    • Promoting independence

    The program cares for patients with conditions such as:
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Tracheotomy
    • Pneumonia
    • Chronic emphysema

The coronavirus pandemic has deeply impacted lives in our local community and lives all around the world. According to the CDC, older adults and people who have severe underlying medical conditions like heart or lung disease or diabetes seem to be at higher risk for developing more serious complications from COVID-19. At American Senior Communities, we are working tirelessly to reduce the spread of this virus and its effects on our residents and employees. This includes aggressive testing. Uniquely and early on, ASC created our own testing strike team of nurses. We partnered with a private lab to expand our testing capabilities to more quickly and thoroughly identify residents and employees that may be affected by COVID-19. Senior communities that test aggressively may report higher number of cases than other communities that do not have the test capabilities of ASC. This strategy enhances our ability to quickly identify and implement isolation protocols as appropriate. Our testing strategy is ongoing and includes retesting residents who are in recovery.

Open communication with families and loved ones is an extremely important priority for us. Below is the most current COVID-19 data for this community.

RESIDENT DATA

  • “Confirmed active cases (residents)” This number indicates the current number of residents who have tested positive. These residents may or may not have symptoms of the illness but have had a laboratory test indicating the presence of COVID-19. These residents are cared for with enhanced infection control techniques by our dedicated staff. Residents who test positive for COVID-19 reside separately from those who test negative to limit the spread of the illness. As each resident’s condition improves, retesting is conducted to confirm their recovery.
  • “New cases in the last 72 hours (residents)” This accounts for the number of residents newly diagnosed.
  • “Deaths related to COVID-19 in the last 24 hours” It is with great sadness that we report any resident we love and care for has passed. The number below indicates the number of residents that passed away in the last 24 hours that were confirmed or suspected  to have had COVID-19. This includes residents with other conditions/diagnoses that may have led to their passing but were also diagnosed with COVID-19.
  • “Total deaths of residents” This indicates the total number of residents that passed away who were confirmed or suspected to have had COVID-19. This includes residents with other conditions/diagnoses that may have led to their passing but were also diagnosed with COVID-19. Our hearts are heavy as we grieve along with their families.
  • “Total recovered residents” This indicates the number of residents who have recovered utilizing the CDC’s symptom or time-based strategy. Under this guidance, 21 days must pass after the first symptom(s) appear or 21 days from the first positive test for those that remain asymptomatic. Additionally, there must be 72 hours with no fever or use of fever-reducing medications. Residents that are asymptomatic but have tested positive for COVID-19 must continue to be symptom free for 21 days before they would be considered recovered. The CDC’s guidance allows for recovery after 10 days. However, for the protection of our residents, we are taking a more stringent approach by waiting 21 days.

  • “New cases of respiratory illness among resident and/or employees in the last 72 hours” A Yes indicates that 3 or more new cases of respiratory illness have occurred in the last 72-hours in residents and/or staff within the community.

  • “Confirmed active cases (employees)” This accounts for the number of employees who have tested positive for COVID-19 and are directed to self-isolate at home.
  • “New cases in the last 72 hours (employees)” This accounts for the number of employees newly diagnosed and are directed to self-isolate at home.
  • “Total recovered employees” This number indicates the total number of employees who, at some point, tested positive for COVID-19 but have since recovered and have been cleared to return to work.
