These surveys include detailed record reviews and conversations with residents, families, and team members, offering a comprehensive picture of each community.
Earning a deficiency-free rating, the highest possible distinction, is a rare and meaningful achievement. It reflects not only strong systems and processes, but a deep, daily commitment to doing what’s right for those we serve.

Even more notably, among those Top 20 providers, ASC achieved the second-highest overall CMS Star Rating in the nation; an affirmation of the consistency and quality of care delivered across our communities.
