A deficiency-free rating is the highest rating a senior community can receive from annual surveys conducted by the Indiana State Department of Health, and only a small percentage of communities achieve this distinction in any given year. Assessment metrics include cleanliness, staffing to patient ratio, 100% compliance with state and federal regulations, safety practices, dietary practices, quality of care, and more. Surveys include unannounced visits by inspectors, record checks, and interviews with residents, family members and staff.
