Moving Forward rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.
Our person-centered program is tailored to the individual. Moving Forward includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.
Exclusively at American Senior Communities, the Road to Recovery program offers the opportunity for our interdisciplinary team, the patient and their family to meet within the first 72 hours after admission to map out their Road to Recovery by identifying discharge goals.
Returning home as quickly as possible is always the desire. However, returning home without the proper rehabilitation can greatly increase the chance of seniors experiencing a setback resulting in rehospitalization. Prior to returning home, our team, in partnership with the patient and family, will conduct a comprehensive in-home safety evaluation to ensure patients can successfully navigate their home with confidence.
No matter what your age or health level, our clinicians are skillfully trained and committed to your success. They take time to get to know and understand what motivates you. Based on proven methodologies, you can count on our clinical team to provide the latest treatments, with respect and compassion. Your health and happiness are always our priority.
Physical Therapy focuses primarily on independent mobility, regaining strength and balance, using proper body mechanics and energy saving techniques to better perform mobility tasks.
Occupational Therapy promotes independence with daily living activities like bathing, eating, dressing, self-care and home management tasks. Participants gain strength and coordination while learning to conserve their energy.
The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.
We monitor our senior rehabilitation outcomes and use the data to consistently improve our services.
Upscale benefits in our Moving Forward wing (at selected locations) include private suites, exclusive dining area and courtyard, electric beds, spacious living rooms, elegant family lounge area, cable TV and phones.
Services include wound care, rehabilitation screenings, fall prevention, nursing, contracture management, wheelchair position, mobility training and walking ability programs. Add this to our highly successful strength-building wellness and aerobic conditioning program, and you have an incredible combination that will truly help you Move Forward!
As experts in memory care, American Senior Communities has reimagined person-centered, life-enhancing care in a way that is engaging, yet safe. With COVID-19 VACCINATION rates increasing and local county rates of infection decreasing, It’s Time. It’s time to consider Memory Care for your loved one and you can do so with confidence at American Senior Communities. To learn more click here.
While a cure for Alzheimer’s disease does not yet exist, many options are available that enable those with the disease or other forms of memory loss to enjoy a higher quality of life. The progression of Alzheimer’s and the symptoms that accompany it often make it difficult for family members to continue care at home. In response, we’ve created Auguste’s Cottage, a program specializing in serving each individual through their unique experience with memory loss.
Auguste’s Cottage is a structured, research-based memory care program. We utilize a person-centered, wellness-based model of care that incorporates stimulating daily activities to sustain independence and promote a healthy spirit. The concept is to focus on maintaining a secure environment where residents have a sense of belonging and meaning each and every day. We offer:
Auguste’s Cottage is a self-contained center within our communities. We surround residents with artwork and special personal items from their past to help foster a sense of belonging and cultivate feelings of security. Plants adorn the halls and pets visit frequently. This protected environment fosters dignity and supports autonomy by reducing challenges.
Long Term Care is a great option for those who struggle with daily tasks and can no longer be cared for at home due to illness, injury, frailty or other limitations. Under the guidance of an attending physician, Long Term Care is provided in our communities by licensed nurses, specially-trained health care aides and a full interdisciplinary care team.
Long Term Care includes a variety of services, including social services and activities as well as physical, occupational and speech therapies to assist our residents in maintaining the highest level of independence possible. Our settings are home-like, cheerful, soothing and secure to ensure the highest quality of life for our residents.
Along with private pay, ASC is proud to accept a wide variety of payment options including Medicaid for qualified applicants. Our Medicaid specialists can assist you and your family through the application process.
While it’s natural for caregivers to allow their own needs to get pushed aside, it’s important for them to realize that they deserve a break from their responsibilities in order to properly care for themselves.
This is where senior respite care can make a difference in a caregiver’s life. Respite stays are an option for those who need respite care services for elderly friends or family on a short-term basis. This allows home caregivers the ability to travel, start a new job, recuperate from an illness, or pursue other interests. Many find respite stays are the perfect option for recovery from surgery or hospitalization.
Should a short stay require extended care, we can accommodate that, too. We carefully examine the needs of our respite guests and provide the level of care required to make their stay comfortable and enjoyable. Family members leave with the confidence that their loved one is safe and in good hands.
Hospice and palliative care for seniors is designed to provide comfort, care, and pain relief for those facing terminal illness. Hospice care involves a team approach to supporting patients and their families with expert medical care and pain management, along with meeting the spiritual and emotional needs of both the patient and the family.
Hospice and palliative care represent a compassionate approach to end-of-life care, assuring the best quality of life and allowing your loved ones to live as comfortably as possible. American Senior Communities partners with local hospice companies to offer comprehensive hospice services, including skilled nursing care, respite care, spiritual and bereavement counseling, medical equipment and supplies, social services, and pharmacy services. By assembling an interdisciplinary team of physicians, nurses, aides, volunteers, social workers and clergy, we are able to serve the special needs of hospice patients and their families.
Outpatient Therapy at American Senior Communities is a personalized approach to clinically-proven strategies that promote overall better health. By combining highly-trained and compassionate therapists with the latest technologies, our programs are structured to fit your needs. Goals are set and guided by therapists who are committed to excellence, integrity, quality and customer service. Our treatments are designed to increase mobility by reducing inflammation and pain. In coordination with your physician, the best, most personalized care for you is always our highest priority.
• Effective, scientifically proven pain management interventions
• The utilization of new technologies that are effective and make therapy fun
• Physician-approved interventions for post-operative orthopedic and other procedures
• We encourage family and caregiver participation

The coronavirus pandemic has deeply impacted lives in our local community and lives all around the world. According to the CDC, older adults and people who have severe underlying medical conditions like heart or lung disease or diabetes seem to be at higher risk for developing more serious complications from COVID-19. At American Senior Communities, we are working tirelessly to reduce the spread of this virus and its effects on our residents and employees. This includes frequent testing for the coronavirus and the administration of the COVID-19 vaccine.
Following the U.S. Food and Drug Administration’s approval of COVID-19 vaccines for emergency use, ASC began administration of the COVID-19 vaccine and is partnering with CVS and their long-term care pharmacy, Omnicare, who bring their expertise in serving seniors. We are thankful for all who worked to bring the vaccine safely to us. The vaccine is being offered free of charge to all residents and staff in two doses, given 28 days apart. Our goal is to have 100% of our residents and staff vaccinated. We have aggressively provided education and counseling to ensure compliance.
Open communication with families and loved ones is an extremely important priority for us. Below is the most current COVID-19 data for this community.
RESIDENT AND EMPLOYEE DATA
Our residents and the families we serve are very important to us. We will continue to move forward with compassion, an abundance of caution and quality care. For general information and updates, visit ASCCare.com/COVID19update.
We are in this together. Thank you for your continued support.
“My sister has been at Bethlehem Woods for six years. They have taken great care of her. I wouldn’t want her in any other facility. They go above and beyond to meet your needs. Thanks for all your care and love you show your residents. It’s a homey atmosphere, and my sister couldn’t be in a better facility.”— Diane Jones, sister of resident
“John has never been at a better place. He’s happy, gets excellent care and is getting along well. He cares about everyone there, and I believe everyone there cares about him. I’m so glad we made the decision to bring him to Bethlehem Woods. From July of 2016 to now, he has done a complete turnaround. He no longer feels isolated or forgotten. The fact that they (the nurses and staff) stop in and check on him has made his life there so wonderful. Thank you to everyone for caring. I am so grateful.”— Linda Reeves, family member
“When both of my parents were diagnosed with dementia and we were unable to take care of them, Bethlehem Woods became my parents’ home. The staff has always been willing to accommodate us and have tried their best to make my parents comfortable and to feel at home. They have supported my family during our tough times of adjustment. We do really appreciate their care and support for my parents. Thank you to all of the staff. May God bless you all!”— Rosemarie Encabo, daughter of residents
“Bethlehem Woods reminds me of a homey cottage. Like a cabin in the woods, it is not fancy or showy but warm and inviting. The staff is always smiling, their voices sweet, gentle and kind. It appears they have been given training in applying informed care. The staff is very responsive and never appear frustrated or overwhelmed. I am sure the work is not easy, though. There is a lot of teamwork among the staff. The staff uses humor to cajole my mother, and she responds positively. The facility is clean. I really like the people at Bethlehem Woods. The executive staff and the caregivers are the best!”— Donna Bolinger, daughter of resident
“I will continue to highly recommend Bethlehem Woods to friends and family! As of yet I have not heard of or observed a facility superior to Bethlehem Woods. They listen to residents and visitors with courtesy and take the time to discuss important matters thoroughly with residents and family. Not all of us are medical professionals, after all! Thank you to all staff and assistants for doing such an excellent and professional job!”— Anita Bowers, resident
